Social Support for People with Memory Problems Living in Torfaen

Torfaen Memory Service have compiled a list of social support available for people with memory problems who live in Torfaen. These include day centres, groups, keeping you company at home, helping you get out, 1 to 1 befriending, and direct payments scheme.

Age Cymru Gwent – general support and advice including benefits / financial assessment. Tel: 01633 238310

Wales Dementia Helpline – offers emotional support. Calls are free and you do not need to provide any personal information. Freephone 08081 410043

Community Connectors – are based at Torfaen Voluntary Alliance and provide advice / signposting regarding a range of community activities. Tel: Leanne Peckham 01495 765646

The Friendly Trust – provides independent advice and practical help with financial matters, including Lasting Powers of Attorney, wills, and Welfare benefits advice. Tel: 02920225200

Community Council – provide a gardening and decorating service. Tel: 01633 624152

Befriending Schemes

Befrienders aim to visit weekly to either sit with you or accompany you in your chosen activity.

  3. Bron Afon – 1 to 1 befriender for those aged 50+ who are experiencing social isolation. Tel: 01633 620340
  4. Cross Roads – provides support in the family home. A sitter service ora 1 to 1 for community activities. Tel: 01495 769996
  5. Side by Side – a volunteer befriender will visit and stay in with you to have a chat and provide company. Contact Sally Guy Tel: 01633 665110

Social Services – can organise a range of day activities including direct payments, help at home or respite care. Tel: 01495 762200 or for out of hours support Tel: 0800 328 4432

Direct Payments – are payments of money made by the local authority to enable you to meet your own support needs by purchasing services. For initial discussion contact Sam Morgan Tel: 01633 746646

TESS (Tenancy Support Service) – organised by Torfaen Voluntary Alliance, they provide community support workers and help with things like moving home, advocacy, confidence building and emotional support. Support id free and lasts for up to 2 years. Tel: 01495 356755
